Transition Care

The NSW Agency for Clinical Innovation (ACI) Transition Care Service assists young people 14-25 years with chronic and complex health conditions as they move from children's to adult health services, there are three area coordinators for NSW.

Transition should be well planned and begin early to ensure an uninterrupted, coordinated, and smooth transition from child centred to adult oriented health care.

What services are provided?

Coordinators work closely with clinicians to make sure all young people have a transition process in place.

Services include:

  • Help work through difficulties in finding or attending adult health services.
  • Provide information about adult health services and how to access them.
  • Provide guidance to attend adult clinics and make sure that young people are successfully linked in to adult health services.
  • Offer support to young people adjusting to new doctor(s) or health service(s)
  • Make sure each young person has a General Practitioner (GP) as this will be the first point of contact for access to most adult services.
  • Keep in contact with the young person transitioning from child to adult services until they are well linked in with their new doctor(s) or health care team.

How do I make an appointment?

Referral to our service can be made by the young person, parent / carer or other family member, paediatric or adult clinical team, nursing or allied health team (physiotherapist, occupational therapist and nutritionist) by completing and sending a Referral to Transition Service form.
