Youth Health Team

 

The Youth Health Team is a multidisciplinary outreach service consisting of Clinical Nurse Consultants, Social Workers, a Staff Specialist Paediatrician and Administration Officer. The Youth Health Team provides services to young people affected by homlessness, or at risk of homelessness, stressful life situations, physical and emotional concerns, relationship issues including domestic violence, at risk behaviours, sexuality concerns, sexual health issues and chronic illness.


What services are provided? 

The Youth Health Team provides sexual health education, contraceptive advice, sexually transmitted infection screening and education, pregnancy testing and pregnancy counselling, post-termination councelling, support with general health issues, counselling for dealing with life situations, referral to other services and service providers and assistance with navigating the health system.

The Youth Health Team provides outreach or centre based appointments.

Do I need a referral and how do I make an appointment?

Referral can be made by the young person, community organisations, school, doctors, parents and any other interested person. For enquiries please call the Youth Health Team office on 49257804 or to make a referral download the Youth Health Team referral form.
